Water Supply Applications
 
 
  A. NEW SERVICE (Occupation Certificate Issued)  

A customer may apply for NEW WATER SUPPLY subject to the following conditions:-
  1. obtaining an approved Application Form / Agreement Form from the housing developer concerned;
  2. completing Form B (Borang Permohonan untuk Bekalan Air) for subsequent processing pertaining to payment of Water Deposit at any Customer Service Counter;
  3. payment of deposit by cash or cheque (payable to PBAPP Sdn Bhd).
 
  B. NEW SERVICE (Private House / Kampung House / Temporary Supply)  

A customer may apply for NEW WATER SUPPLY subject to the following conditions:-
  1. obtaining an Application For Water Supply Form (Form A) and (Form B) from any Customer Service Counter;
  2. obtaining the services of any certified plumber who will complete (Form A) and (Form B) enclosing a construction plan for action by the Operations Department (Services Section);
  3. On approval, the customer may personally or through the plumber make payment for Trunk Mains Contribution, Piping Deposit and, if necessary, Road Reinstatement Charges;
  4. Payment may be made in cash or cheque (payable to PBAPP Sdn Bhd).
 
  C. RECONNECTION  

A customer may apply for reconnection of water supply subject to the following conditions:-
  1. submitting a copy of Sales and Purchase Agreement (not more than 5 years), or latest Assessment Bill issued by the Majlis Perbandaran;
  2. completing (Form B) and paying the Water Deposit at any Customer Service Counter;
  3. Payment may be made in cash or cheque (payable to PBAPP Sdn Bhd).
  4. Settling any reconnection costs or meter charges or any other relevant charges, if any.
 
  D. CHANGE OF NAME  

A customer may apply for water supply through change of name, subject to the following conditions:
  1. submitting a copy of Sales and Purchase Agreement (not more than 5 years) or latest Assessment Bill issued by the Majlis Perbandaran;
  2. completing (Form B) and paying the Water Deposit at any Customer Service Counter;
  3. Payment may be made in cash or cheque (payable to PBAPP Sdn Bhd);
  4. Settling any arrears of payment, if any, of the previous consumer.
